การพัฒนารูปแบบการสร้างปฏิสัมพันธ์ระหว่างพยาบาลกับผู้รับบริการเพื่อเสริมสร้างการปฏิบัติที่ดีในการรักษาพยาบาลของโรงพยาบาลของโรงพยาบาลศูนย์ สังกัดกระทรวงสาธารณสุข

Abstract

การวิจัยคร้ังนี้เป็นการวิจัยและพัฒนา มีวัตถุประสงค์ 1) เพื่อศึกษาข้อมูลพื้นฐานและสภาพการณ์ระหว่าง พยาบาลกับผู้รับบริการ ของโรงพยาบาลศูนย์ สังกัดกระทรวงสาธารณสุข 2) เพื่อพัฒนารูปแบบการสร้างปฏิสัมพันธ์ ระหว่างพยาบาลกับผู้รับบริการ 3) เพื่อทดลองใช้รูปแบบ และ 4) เพื่อประเมินและถอดบทเรียนรูปแบบ ผู้ให้ข้อมูลหลักในการสัมภาษณ์เชิงลึก เลือกแบบเจาะจงจากพยาบาล 5 คนและผู้ป่วย 5 คนของโรงพยาบาลศูนย์ตัวแทนจาก 5 ภาค ภาคละ 1 แห่งรวม 50 คน กลุ่มเป้าหมายในการทดลองใช้รูปแบบเลือกแบบเจาะจงจากพยาบาล 15 คนและผู้ป่วย 15 คนของโรงพยาบาลศูนย์ ที่เป็นตัวแทนในการทดลองใช้ รวม 30 คน เครื่องมือที่ใช้คือ แบบทดสอบก่อนและหลังอบรม แบบประเมินผลการอบรม แบบประเมินความพึงพอใจของผู้เข้ารับการอบรม วิเคราะห์ข้อมูลเชิงปริมาณด้วยสถิติคือ ความถี่ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน วิเคราะห์ข้อมูลเชิงคุณภาพด้วยการวิเคราะห์เนื้อหา ผลการวิจัยพบว่า 1)ข้อมูลพื้นฐานและสภาพการณ์ระหว่างพยาบาลกับผู้รับบริการของโรงพยาบาลศูนย์สังกัด กระทรวงสาธารณสุขพยาบาลควรมีคุณธรรม จริยธรรม และคำนึงถึงผู้ป่วยเป็นศูนย์กลาง ในการให้การพยาบาล ส่วนผู้รับบริการควรมีความเห็นใจ และปฏิบัติตามคำแนะนำต่างๆ ด้วยความเคารพกฎระเบียบของโรงพยาบาล 2)ได้ รูปแบบชื่อว่า “PICT Model”ประกอบด้วย (1) พยาบาลกับผู้รับบริการมีส่วนร่วมในการสร้างปฏิสัมพันธ์ให้เกิดขึ้น (P=Participation) (2)พยาบาลกับผู้รับบริการมีการบูรณาการการปฏิสัมพันธ์ที่ดีมาไว้ร่วมกัน (I=Integration) (3)พยาบาลกับผู้รับบริการมีการประสานความร่วมมือที่ดีต่อกัน (C=Coordination) และ(4)การฝึกการปฏิบัติกิจกรรมการพยาบาลต่าง ๆ ร่วมกัน (T=Training) 3)ผลการทดลองใช้รูปแบบพบว่ากลุ่มเป้าหมายมีผลสัมฤทธิ์การเรียนรู้หลัง อบรมสูงกว่าก่อนอบรมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ส่วนประโยชน์ที่ได้จากการอบรมคือได้มิตรภาพ ความเป็นกัลยาณมิตร และได้แนวทางการพัฒนาการสร้างปฏิสัมพันธ์ระหว่างพยาบาลกับผู้รับบริการของโรงพยาบาลศูนย์สังกัดกระทรวงสาธารณสุข และ 4)ผลการประเมินและถอดบทเรียนรูปแบบพบว่าผู้ร่วมกิจกรรมพึงพอใจรูปแบบทุกด้านในระดับมากและถอดบทเรียนได้ 4 บทเรียนคือ (1)การมีส่วนร่วมที่มีพลังมาจากสมาชิกทุกคนที่เข้ามาช่วยร่วมกันในการขับเคลื่อนกระบวนการพัฒนา สามารถทำให้กิจกรรมเดินหน้าสู่ความสำเร็จได้ (2)การพัฒนารูปแบบในช่วงแรกควรเป็นแบบค่อยเป็นค่อยไปตามกระบวนการ โดยมุ่งไปสู่การมีปฏิสัมพันธ์ที่ดีต่อกันระหว่างพยาบาลกับผู้รับบริการที่ยั่งยืน (3)การจัดการความรู้ของกลุ่มจากการเสวนากลุ่มย่อย การแลกเปลี่ยนเรียนรู้ การจัดประชุมนำเสนอทำให้เกิดการเปลี่ยนผ่านความรู้ท้ังตัวพยาบาลและผู้รับบริการ จนเกิดผลสำเร็จแห่งการมีปฏิสัมพันธ์ที่ดีต่อกันระหว่างพยาบาลกับผู้รับบริการและ(4)ผลของการดา เนินกิจกรรมที่ได้คือมีการขับเคลื่อนกิจกรรมการเรียนรู้และการปฏิบัติต่าง ๆ อย่างชัดเจน สามารถใช้เป็นแนวทางให้แก่โรงพยาบาลศูนย์อื่น ๆ ในลักษณะของการถ่ายทอดความรู้ ขยายแนวคิด และพัฒนากิจกรรมเพื่อการปฏิบัติร่วมกันต่อไปได้ The study was conducted by using the Research and Development Method. The purposes of this research was to 1)study the current conditions and needs of interaction between nurses and patients; 2)develop model of interaction between nurses and patients; 3)experiment model; and 4)evaluate and lessons learned form model. Samples for studying became population by purposive selection. Questionnaire is used for collecting quantitative data by Indepth Interview from nurses and patients 50 persons in five places of the tertiary hospitals under the ministry of public health. Target groups became the resulted of nurses and patients 30 persons in one place of the tertiary hospitals under the ministry of public health. The instruments pre test and post test, Training Evaluation Form and Satisfaction Evaluation Form. Analyzing Data by Quantitative data analysis with statistical frequency, average and standard deviation and qualitative data were analyzed using content analysis. The findings of the research 1)the basis and conditions of service of nurses to the tertiary hospitals under the ministry of public health. should have both a moral and ethical provide appropriate patient care and regard for patient centered care in providing and the service should have a compassionate way follow the instructions with respect to the rules and regulations of the hospital. 2)forms"PICT Model"include:(1)nurse and patients are involved in the interaction to occur (P=Participation) (2)nurses with patients integrating the good interaction it together (I=Integration) (3)nurses and clients have cooperated well with each other (C=Coordination) and(4)activities for the nurse to patient studies knowledge sharing (T=Training). 3)the results take the form states found that the achievement of learning goals for the higher education level of statistical significance. 05 and the benefits of the training are friendship, goodwill and learn ways to improve the interaction between nurses and patients of the tertiary hospitals under the ministry of public health. And 4)the results of the activity is driven learning activities and practices can be adopted to other hospitals and kind of knowledge transfer expand the concept and development activities for the common practice of the situation.
