ปฏิกิริยาทางผิวหนังชนิดรุนแรง (Severe cutaneous adverse reactions; SCARs) เป็นปฏิกิริยาที่สามารถส่งผลให้เกิดการบาดเจ็บ การนอนโรงพยาบาล และการเสียชีวิต การเกิด SCARs ในผู้ป่วยวัณโรคมีผลให้ผู้ป่วยต้องหยุดยา และส่งผลสืบเนื่องทำให้การรักษาล้มเหลว มีการแพร่กระจายเชื้อไปสู่ผู้อื่น และเพิ่มโอกาสการเกิดเชื้อดื้อยาได้  งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาอัตราการเกิด SCARsจากยาต้านวัณโรคทางเลือกแรกในประเทศไทยวิธีการศึกษาเป็นการทบทวนย้อนหลังข้อมูลปฏิกิริยาข้างเคียงจากยาต้านวัณโรคที่เกิดเหตุการณ์ตั้งแต่ 1 มกราคม 2555 ถึง 31 ธันวาคม 2559  ซึ่งข้อมูลดังกล่าวได้รับจากศูนย์เฝ้าระวังความปลอดภัยด้านผลิตภัณฑ์สุขภาพ สำนักงานคณะกรรมการอาหารและยา กระทรวงสาธารณสุข กลุ่มศึกษาคือรายงานอาการ    ไม่พึงประสงค์ของผู้ที่ใช้ยาต้านวัณโรคทางเลือกแรก ได้แก่ Isoniazid (INH), Rifampin (RMP), Ethambutol (EMB), Pyrazinamide (PZA), Streptomycin (Sm) และต้องเป็นรายงานที่ถูกประเมินว่าเกิด SCARs อย่างใดอย่างหนึ่งดังต่อไปนี้ Stevens-Johnson syndrome (SJS), Toxic Epidermal Necrolysis (TEN), Drug Reactions with Eosinophilia and Systemic Symptoms (DRESS) และ Acute Generalized Exanthematous Pustulosis (AGEP)  ผลการศึกษาพบรายงานผู้ป่วยเกิด SCARs จากยาต้านวัณโรคทางเลือกแรก 229 คน คำนวณอัตราการเกิด SCARs ได้ 5-6 คนต่อผู้ป่วย 10,000 คนต่อปี SJS เป็น SCARs ที่พบสูงสุด ประมาณการอัตราการเกิดอยู่ระหว่าง 6-8 คนคนต่อผู้ป่วย 10,000 คนต่อปี ขณะที่ TEN, DRESS และ AGEP สามารถประมาณการอัตราการเกิดได้ 1-3, 1-3, และ 0-2 คนต่อผู้ป่วย 10,000 คน ต่อปีตามลำดับ  ผู้ป่วยส่วนใหญ่เป็นผู้ป่วยใน โดยพบว่าประเภทของผู้ป่วย (ผู้ป่วยในหรือนอก) มีความสัมพันธ์กับผลลัพธ์ ภายหลังเกิด SCARs (p = 0.017)   RMP และ INH  เป็นยาสาเหตุหลักของการเกิด SCARs ในผู้ป่วยโดยพบ ร้อยละ 23.30 และ 21.36 ตามลำดับ ขณะที่ EMB, S, PZA พบร้อยละของการเกิด SCARs ร้อยละ19.42, 19.42, และ 16.50 ตามลำดับ  ระยะเวลาตั้งแต่ได้รับยาจนเกิด SJS , TEN, DRESS, และ AGEP  มีมัธยฐานที่ 15, 36,31และ 22 วันตามลำดับ  พิจารณายาต้านวัณโรคทางเลือกแรกแต่ละตัวที่เป็นสาเหตุการเกิด SJS พบว่า มัธยฐานระยะเวลาตั้งแต่ได้รับ INH, RMP, EMB, PZA,และ Sm จนเกิด SJS เท่ากับ 12, 6, 19, 11 และ 14 วันตามลำดับ โดยระยะเวลาตั้งแต่ได้รับ RMP จนเกิด SJS สั้นกว่ายาต้านวัณโรคตัวอื่นอย่างมีนัยสำคัญ (p=0.014)

Warfarin is oral anticoagulant that has been used for prevention of systemic thromboembolism in valve replacement patients. There are many factors that can change the international normalized ratio (INR) in warfarin users. This study aimed to determine the influence of factors that related to the target range of INR among warfarin users who underwent heart valve replacement. This study is a retrospective study. The data were collected in patients who underwent valve replacement in warfarin clinic, Phramongkutklao Hospital, Thailand from January 2007 to December 2015. Demographic data including gender, age, type of prosthetic valve, co-morbidities, other medications used with warfarin, herbs or dietary supplements used with warfarin, laboratory data, warfarin initial dosing regimen and INR value were collected. The demographic data and the risk factors were analyzed via descriptive statistics and Fisher's exact test consecutively by using R-program. One hundred and nineteen patients were included. Sixty-four cases were male (53.78%) and mean (±SD) age was 55.09±14.78 years old. The mean (±SD) of body weight was 61.48±13.97 kilograms. Of those, 96 cases (80.67%) had mechanical valve replacement. Only sixteen of warfarin users (13.45%) had INR values with in target range. The results of the present study showed that drug interaction was one of the important factors that was associated with out-of-range INR target (OR 10.17, 95%CI 0.59-175.77) (p=0.051). Drug interaction including drug-drug, drug-herbs and drug-dietary supplement interaction were concerned in patients who received warfarin underwent heart valve replacement

Candidemia is a major cause of morbidity and mortality among hospitalized patients in Thailand. In this study we aimed to retrospectively investigate the clinical outcomes by treatment regimen and the factors associated with mortality among patients with candidemia at a hospital in Thailand in order to inform future treatment strategies. Inclusion criteria for study subjects were patients admitted to Phramongkutklao Hospital during January 2016-December 2019 who had laboratory confirmed candidemia. Exclusion criteria for subjects were: patients with candidemia who had a positive fungal blood culture prior to 48-hours hospitalization, who had incomplete medical records or who were referred to another hospital. The medical records of each subject were reviewed and the following recorded: demographic data, antifungal regimen, length of hospitalization, Candida species identified and treatment outcome. A total of 159 subjects were included in the study; 57.9% male. The median (interquartile range) age of subjects was 70 (58-81) years. Of the 159 subjects, 81.8% received antifungal therapy: amphotericin B in 67.9%, echinocandins in 5.7%, and fluconazole in 8.2%. The overall mortality rate among the 159 subjects was 72.3%, the mortality rate among the subjects with no antifungals was 100%, and the mortality rate among those treated with antifungals was 54.1%. The mortality rates among those treated with amphotericin B, echinocandins, and fluconazole were 64.8%, 44.4%, and 92.3%, respectively. The factors significantly positively associated on multivariate analysis with an increase in mortality at 30 days hospitalization were: shock (odds ratio (OR): 6.29; p-value &lt;0.001), endotracheal intubation and mechanical ventilation (OR: 4.11; p-value = 0.017) and disseminated intravascular coagulation (OR: 6.05; p-value = 0.038). The factors significantly negatively associated with increased mortality at 30 days hospitalization were: the use of amphotericin B (OR: 0.34; p-value = 0.035) and the use of echinocandins (OR: 0.03 p-value = 0.002). In our study, there was a high mortality rate even when treated, especially among those with shock, those who were intubated and those who had disseminated intravascular coagulation. Our results show that early treatment with either amphotericin B or echinocandins, but not fluconazole, were associated with improved mortality. Further studies are needed to determine if application of these factors in the treatment of these patients can improve mortality at the study institution

This study aimed to determine the prevalence and antimicrobial susceptibility of causative pathogens associated with 30-day mortality among intensive care unit (ICU) patients with bloodstream infections as well as to analyze clinical outcomes and risk factors. This retrospective study was conducted at Phramongkutklao Hospital, Thailand, between October 2017 and December 2018. The study included 142 patients with bloodstream infections caused by 193 isolated pathogens. Of the 142 patients, the clinical cure and death rates were reported to be 40.1% and 62.0%, respectively. Of the 193 isolated bacterial strains, 83.4% were Gram-negative: predominately Klebsiella pneumoniae, followed by Acinetobacter baumannii, and then Pseudomonas aeruginosa isolates. Gram-positive bacteria accounted for 16.6% of the total isolated bacterial strains: Staphylococcus aureus was the leading isolate, followed by coagulase-negative staphylococci and then Enterococcus faecalis. Most of the A. baumannii isolates (97.0%) were resistant to meropenem, but 57.7% of P. aeruginosa and 40.0% of K. pneumoniae were susceptible to meropenem. Through multivariate analysis, it was found that the significant factors for 30-day mortality were male gender, catheter-related bloodstream infection, and carbapenem-resistant Gram-negative infection. Due to the high 30-day mortality rate of almost two-thirds of the study groups, healthcare professionals are challenged to select an optimized treatment regimen for ICU patients with carbapenem-resistant gram-negative infection
